I had the same problem... Now it is gone, G4 400 yikes, 8.6 OS. In the Extension Manager I used Mac O.S. Base set extensions and added OMS, Digidesign, and USB extension (USB-2-Midi)... Renamed the system to Pro-tools base set..... No more problems.... Ohhh and I hate my mouse so I got a USB trackball... I'm Happy now

I am using os9 with g4 450 and 256 ram with a atto scsi card and ultra wide drive 10,000 rpm. My scsi card is in the first slot and the digi card is in the 3 rd. slot.Called tech support and they told me to disconnect all ati extentions.WHO KNEW !!!!!!!!! ANY INFO WOULD HELP SPIDEE
